4 easy steps to earn your Surgical Services Educator Certificate of Mastery
To earn your certificate of mastery and continuing education for the Surgical Services Educator program, follow these steps.
1. Review the certificate program packet to become familiar with the details of the program. A Certificate Program Packet will be included in your binder, you may also access individual forms on the links below. The packet includes:
- Description of activities and rubric
- Application form
- CE registration form
- CE evaluation form
2. Read the modules. It will be difficult to complete the activities without this information.
3. Complete the activities. Each module has 2-3 activities which, when completed, help to illustrate integration of the content of the modules to actual practice. Activities from all 7 modules must be completed to receive the certificate of mastery and continuing education.
4. Submit the following documents:
- Completed application form. The person purchasing the set, or his/her designee, will be the only person accepted as the certificate applicant.
- Completed activities for all 7 modules. Except where noted, activity must be typed. Partial credit for activity or continuing education cannot be given for individual modules.
- $35 processing fee
- CE Registration form
- CE Evaluation form

To receive contact hours, in addition to completing the activities at the end of each module, complete the registration and evaluation forms and submit them with the rest of your packet.
