Practice Hour Database
To access your practice hour database, you must first logon through My Certification.
Duplicate Certificate
For a $10 fee, plus $2.95 for shipping and handeling, you may request a reprint of your certificate by clicking on the button below, or calling the CCI office at (888) 257-2667. The process takes approximately two to three weeks, since we must send the request to an outside vendor for printing.
