Application for CRNFA Recertification by Contact Hours

Begin the online CRNFA recertification application here. If you're recertifying and have been selected for an audit, you can fill out this application, attach the additional information requested, and mail or fax to us. You may also elect to recertify using our paper application.

If you work for the VHA or DOD, you can receive a discount on certification. Please visit our "Let's Get Certified Page."

AORN DISCOUNT: To receive your AORN membership discount, please fill in the AORN application fields. If your information is incorrect, your discount will not be applied automatically.

		I am currently certified as a CRNFA.

		I am currently certified as a CNOR or a nurse practioner certified in my specialty.

		I am currently licensed as an RN, without provision or condition, in the country where I am currently practicing.

		I have practiced as a part-time or full-time RNFA at some time within the two years immediately prior to application. Required practice hours must have been accumulated in the last 5 years.

		I have completed at least 1000 documented hours of practice as an RNFA (of which 700 were in intraoperative patient care) including pre-, intra- and postoperative patient care. (The pre- and postoperative patient care does not necessarily have to be connected to cases on which the applicant has assisted.)

A certain percentage of our members are randmonly selected to have their applications audited. If you have been selected for an audit you would have received a notification letter with the application packet that was mailed to you. Please indicate above if you were selected for an audit.

Practice Hours Requirement
		1000 Practice Hours / 200 Contact Hours I have completed at least 1000 practice hours in the past five years and have at least 200 contact hours.

		500 Practice Hours / 300 Contact Hours I have completed at least 500 practice hours in the past five years and have at least 300 contact hours.
